Good news, I can help! It sounds like your 2012 Chevrolet Express Van didn't come with the tow package. This means that there isn't a OEM factory plug for the # HM40975-11933 to plug into. If your Express Van had been built with the tow package there will be a plug at the back of the vehicle where, yes, the Hopkins 4 & 7-way plug will just simply plug into.

While I do have a possible solution for you, there aren't any custom fit front mount hitches designed for your 2019 Chevy 4500 Cutaway Chassis so for your application you'll need to use an adjustable hitch like the Curt # C13701. This will require you to trim the crossbeam of the Curt hitch as well as figure out how to fit it to your Chevy so if this isn't something you're comfortable with I recommend having a shop help you out with it.

The brake signal output wire you will want is the yellow wire on your 2019 Chevy Express 3500. This wire should only be energized when the brake pedal is pressed.
